In April of 2020, at the request of the FDA, Zantac and ranitidine manufacturers initiated one of the largest pharmaceutical recalls in American history. Both independent and government testing revealed levels of the carcinogen, NDMA, that warranted the recall due to the threat to consumer health. The recall included over-the-counter and prescription Zantac (name-brand and generic ranitidine).

NDMA, nitrosodimethylamine, the contaminant in Zantac and generic ranitidine, is a known carcinogen. Once used as a propellant in rocket fuel, NDMA is so toxic its only permissible use in the U.S. is to cause cancer in lab rats. NDMA dissolves easily in water and its documented use in intentional poisonings dates back to at least 1973.
It is still unclear exactly how the medication became contaminated with NDMA. Suspected causes of the contamination include heating during transportation or digestion creating a chemical process that produces NDMA. Others have speculated that when the active ingredient in Zantac, ranitidine, comes in contact with water a chemical reaction produces the NDMA.
